How. Written information goes in within three days of the notice of administrative revocation. If the review then revokes, a hearing request form and a $30 fee must reach the ADLRO within six days of that decision being mailed. The fee can be waived.

If it passes. A request between six and sixty days still gets a hearing, but the temporary permit is not extended — so you are off the road while you wait. Past sixty days, generally nothing is scheduled.
